File Generation

I have a file that i have to use to configure a device. I need a form in which i have to insert the data i need to change in that file for every device so when i click on something like “generate” a script should generate the new file based on the file i attached.For each device i have a different MAC ADDRESS, USERNAME1,PASSWORD1, USERNAME2,PASSWORD2.

The only things to be changed in the file so the only things for which i want to be created a text box where to insert the new values:

Profile_Rule: Line 107 Col 50-58 (mac_here)

Display_Name[1]: Line 328 50-58 (id1_here)
User_ID[1]: Line 329 Col 50-58 (id1_here)
Password[1]: Line 330 Col 50-60 (pass1_here)
Auth_ID[1]: Line 332 Col 50-58 (id1_here)

Display_Name[2]: Line 570 50-58 (id2_here)
User_ID[2]: Line 571 Col 50-58 (id2_here)
Password[2]: Line 572 Col 50-60 (pass2_here)
Auth_ID[2]: Line 574 Col 50-58 (id2_here)

for display_name,user_id,auth_id i need only one form because i have to insert the same value.

So i open the page or the form in Mozilla and I insert MAC in the box called MAC, Username1 in the box called Username1, Password1 in the box called Password1, Username2 in the box called Username2, Password2 in the box called Username2.

Once i click on Generate the script asks me to save the file that i`m sending in the attachment with the new values. The name of the script should be in this format:
mac_address.txt (000a09754a0a.txt)
